西方天竺妙庄王膝下三公主妙善，性慈悲，具慧眼。妙庄王灭毗卢国，残忍对待战俘，妙善率战俘登彩虹而渡河。妙庄王因怒成疾，医者谓非有妙善的双眼双手合药，方能治愈，妙善挖目断臂，王服下竟药到病除。王亲率百官徒步朝山时，妙善已圆寂，化成“千手千眼观音”法相。
